City and County Healthcare Group Ltd in Cardiff is seeking a dedicated Care Coordinator to help organise and deliver high-quality home care across the community.

You will coordinate care referrals, create rotas, liaise with social workers and GPs, and support care workers to maintain exceptional service levels.

The role runs Monday to Friday 9am–5pm with on-call duties on a rota, and a salary of £26,162. A driving licence is required.
