Job summary

CCH Group in Blackburn is seeking a Care Coordinator to organise and deliver high-quality home care in your community. You will manage care workers, assess and review care plans, and support staff in the field.

The role requires a valid driving licence and access to your own vehicle due to travel. Monday to Friday, 9am - 5pm with on-call duties on a rota.

Qualifications

• Experience & Organisation: You are an efficient, organized Care Coordinator who can manage rotas and build strong relationships with care workers.
• Familiarity with CQC requirements and care planning is desirable.
• NVQ level 3 is desirable.

What you’ll do

The Care Coordinator plays a key role in ensuring the highest quality care services by managing care workers and performing field-based duties. Responsibilities include assessing and reviewing individual care plans, as well as supervising, instructing, and supporting care staff in the field. This role requires a valid driver’s license and access to your own vehicle due to the travel involved.
