Job summary

Barrow-in-Furness Integrated Care Community team comprises health and care workers, voluntary organisations, and community assets working together to practice population health. An opportunity has arisen for a Care Coordinator to join the team. We seek a friendly and proactive individual to assess, plan, implement, and evaluate evidence-based patient care, liaising with the multidisciplinary team within a community setting to support residents in living independently. The team develops and implements comprehensive information, education, and support packages for patients and their carers/relatives, including participation in local public events and health festivals. The role offers opportunities to develop and maintain clinical expertise. We offer flexible working hours (....hrs), with options for job share/PT, and welcome Allied Health Professional applicants. Based in Barrow with travel across Furness. Newly appointed staff will undergo a 6-month probationary period to familiarize with the role and environment. For more details or informal visits, contact:
