The Barrow-in-Furness Integrated Care Community team comprises health and care workers, voluntary organizations, and community assets working together to promote population health. We are seeking a Care Coordinator to assess, plan, implement, and evaluate patient care, liaising with the multidisciplinary team in a community setting to support residents in maintaining independence. The role involves developing educational and support packages for patients and carers, with opportunities to develop clinical expertise. Flexible work options are available, including part-time and job share; Allied Health Professionals are welcome to apply. The position is based in Barrow with travel across Furness. New appointees will undergo a 6-month probation period.
Our hospitals include Furness General Hospital, Royal Lancaster Infirmary, and Westmorland General Hospital, along with community healthcare premises across Morecambe Bay. We offer a range of hospital and community services including emergency, outpatient, diagnostics, and outreach services.
