Job overview

The Barrow-in-Furness Integrated Care Community team comprises health and care workers, voluntary organizations, and community assets working together to promote population health. We are seeking a Care Coordinator to assess, plan, implement, and evaluate patient care, liaising with the multidisciplinary team in a community setting to support residents in maintaining independence. The role involves developing educational and support packages for patients and carers, with opportunities to develop clinical expertise. Flexible work options are available, including part-time and job share; Allied Health Professionals are welcome to apply. The position is based in Barrow with travel across Furness. New appointees will undergo a 6-month probation period.

Main duties of the job
  • Assess, plan, implement, and evaluate evidence-based patient care in collaboration with the multidisciplinary team.
  • Develop knowledge and skills in the specialty, including self-management and signposting to community support.
  • Provide health screening clinics in the community, including monitoring long-term conditions and falls prevention, with medical advice available remotely.
  • Ensure accurate documentation of all patient care activities.
  • Manage patient referrals and discharge planning in coordination with other professionals and agencies.

  • Develop clinical expertise and act as a resource within the specialty.
  • Maintain effective communication with patients, carers, and professionals.

  • Use resources efficiently and develop policies/protocols with the lead clinician.
  • Support and mentor less experienced staff and students.
  • Maintain competence in clinical skills and promote health education.
  • Coordinate high-quality patient care and ensure needs are met.
