Role Description

This full-time role for a care coordinator is located on-site in Cumbria. The care coordinator will be responsible for managing and coordinating care plans for clients, liaising with healthcare professionals and families, and ensuring that clients receive timely and effective care. Day-to-day tasks include assessing client needs, organizing and scheduling carers, maintaining detailed records, providing customer service, and ensuring compliance with health and safety regulations.
