Job summary

HCRG Care Group in Gravesham is seeking a Care Coordinator to join the Care Co-ordination Centre at Gravesham Community Hospital. You’ll be the first point of contact for service users, GPs and Integrated Health and Social Care teams, handling referrals and ensuring information reaches the right services quickly.

This role suits someone with strong admin and customer service experience who works well in a team and stays calm in a busy environment.

Qualifications

• Previous administrative experience within a busy, customer-facing environment.
• Excellent verbal and written communication skills.
• Excellent customer service skills.
• A professional and confident telephone manner.
• Strong organisational skills with the ability to manage competing priorities.
• Good attention to detail and the ability to accurately record information.
• The ability to work effectively as part of a team.
• A flexible and adaptable approach to your work.
• Confidence using IT systems.
• A working knowledge of Microsoft Office packages, including Outlook, Word and Excel.
• The ability to handle confidential and sensitive information professionally.

Responsibilities

• Act as a first point of contact for service users, GPs and Integrated Health and Social Care teams.
• Receive and process referrals accurately and within required timeframes.
• Ensure referrals and information are directed to the appropriate services and teams.
• Respond professionally and helpfully to telephone and other enquiries.
• Communicate effectively with patients, healthcare professionals and colleagues.
• Maintain accurate and up-to-date information on relevant systems.
• Provide administrative support to help ensure the efficient day-to-day running of the service.
• Manage competing priorities within a busy Care Co-ordination Centre.
• Handle sensitive and confidential information appropriately.
• Work collaboratively with colleagues to provide a responsive and efficient service.
• Provide a high standard of customer service at every point of contact.
• Identify queries or concerns that require further support and elevate them appropriately.
• Work flexibly across the service’s operating hours, including weekends and Bank Holidays.
