Job summary

Parkside is recruiting a Care Co-ordinator to join our North West London specialist healthcare team in Harrow. The role focuses on coordinating care pathways, triaging referrals with nurses, and acting as the primary contact for patients and professionals to ensure timely, person-centred support.

You will work closely with NHS and community partners in a busy 24/7 service, contributing to high-quality palliative care and reducing avoidable hospital admissions while supporting patients to remain

Responsibilities

• Coordinating referrals and supporting patient care pathways.
• Triaging and prioritising referrals with the nursing team.
• Acting as a key contact for patients, families and professionals.
• Supporting a 24-hour advice and support service.
• Facilitating communication between healthcare providers.
• Maintaining accurate records of referrals.
• Providing information and signposting to appropriate services.
• Supporting efforts to prevent avoidable hospital admissions.
• Collaborating with NHS 111, ambulance teams, GPs and hospice services.
