Job summary

Qualifications

• Assess, plan, implement and evaluate safe and high-quality patient care.
• Lead clinical care for a defined group of patients.
• Work across the full care continuum for arrhythmia management.

Responsibilities

• Develop and improve diagnostic, referral and treatment pathways for arrhythmia patients.
• Collaborate with colleagues across the wider network to deliver high-quality, coordinated patient care.
• Contribute to reduction of waiting times and treatment waiting lists.
• Minimise unnecessary admissions to acute hospital beds.
• Support timely and streamlined care for patients with arrhythmias.
