An exciting opportunity has arisen for a dynamic and innovative nurse to join NHS Fife’s Community Cardiac Specialist Nurse Team as a Cardiac Clinical Nurse Specialist.
The Cardiac CNS Service operates Monday to Friday during business hours. We provide holistic and evidence-based specialist care to people diagnosed with various cardiac conditions in Fife. We have two bases: Stratheden Hospital and Lynebank Hospital. You will be based at Stratheden but will work across Fife as required. You will participate in the ongoing development, implementation, and evaluation of the service to improve management, treatment, and support for patients with cardiac conditions, and their carers and families. The service aims to support self-management, improve quality of life, and reduce unnecessary hospital admissions and re-admissions. It provides seamless care between primary and acute care settings.

Requirements include excellent communication skills, ability to work autonomously as part of the wider Health and Social Care Team, utilizing your own initiative, clinical judgment, and decision-making skills.
This is a Fife-wide community post, requiring reliable and efficient travel across various sites, bases, and patient homes across Fife.
