Job summary

United Lincolnshire Hospitals NHS Trust seeks a Cancer Navigator based at Lincoln County Hospital to coordinate patient pathways, schedule investigations and treatments, and monitor waiting times against national standards. The post involves working with medical staff and partner organisations to ensure high-quality, patient-centred care for those on a cancer pathway.

You will identify potential breaches, take positive action to mitigate them, and support division-wide cancer service delivery

Responsibilities

• Navigate and track patients along their pathway.
• Coordinate hospital appointments and investigations.
• Identify potential breaches of waiting times and take corrective action.
• Engage with senior medical staff, nursing staff and other health professionals.
• Work with partner organisations within the network across the patient journey.
• Analyse problems and propose suitable solutions to workflow gaps.
• Prioritise tasks and manage own time to meet deadlines.
• Maintain patient confidentiality at all times.
• Use MS Office tools for reporting and documentation.
