Mid and South Essex NHS Foundation Trust in Chelmsford is seeking a Cancer Patient Navigator to support the cancer care pathway. Based primarily at Broomfield Hospital, you will coordinate patient journeys, record accurate information in patient records and Trust systems, and work with clinical teams to streamline processes for patients and carers.

You will need excellent communication and organisational skills, knowledge of cancer pathways, and experience with hospital administrative systems

Qualifikationen

• Knowledge of specialist functional and medical terms.
• Previous administrative experience within a healthcare setting.
• Experience of applying hospital administration systems.
• Knowledge and understanding of key access targets i.e. 31 day and 62 day cancer targets.
• High level of proficiency in MS Office.
• Experience with data input.
• Understanding purpose of PTLs (Patient Tracking Lists).

Aufgaben

• Capture and update patient information in records and Trust systems.
• Maintain knowledge of patient complete pathways and ensure information is current.
• Support clinical nurse teams and report to cancer business partners to transform processes.
• Provide cross-site support to ensure consistent and effective support for all cancer sub specialties.
