Job summary

The Transfer of Care Hub is a newly developed team within BSW, responsible for patient flow across three work streams: front door referrals, clinically ready for discharge, and out of area placements. This is a rare opportunity to work as a Bed Capacity Co-ordinator on a 12‑month secondment.

• Support the effective and efficient use of inpatient beds.
• Liaise with clinicians across BSW and external stakeholders to ensure optimum bed utilization.
• Monitor the flow of inpatient admissions and coordinate timely return of out-of-area placements to minimise financial impact.
• Contribute to development of new ways of working within a changing commissioning landscape.
