Job summary

Service Care Solutions Ltd is seeking an Agency Experienced Hospital Social Worker to join the Transfer of Care Hub at our client Hospital on a temporary contract. This fast-paced role focuses on safe, timely discharges with hybrid working (three days in the office).

You will conduct assessments, collaborate with multidisciplinary teams, and support patients and families through discharge while maintaining documentation and safeguarding procedures.

Qualifications

• Recent hospital or Transfer of Care experience is essential.
• At least 3 years of social work experience with referees.
• Strong assessment, communication and decision-making skills.
• Familiarity with social care systems such as Mosaic is desirable.

Responsibilities

• Conduct comprehensive assessments to determine patients' social care needs.
• Collaborate with multidisciplinary teams to facilitate safe and timely discharges.
• Support patients and families through discharge ensuring needs are met.
• Reduce delays in transfers of care by managing caseloads proactively.
• Maintain accurate documentation and safeguard procedures.
