Job summary

Agincare Weymouth is seeking an Outreach Support Worker to assist individuals with learning disabilities, autism and mental health needs in Bromley, Bexley and Newham. You will provide outreach support in clients’ homes and local communities, helping them pursue personal goals and life skills while maintaining meaningful connections.

The role offers £13.00 per hour, with opportunities for training, career development and pension benefits.

Responsibilities

  • Support people in daily living activities including meals, routines and budgeting.
  • Encourage independence and community participation.

  • Work as part of a team and also use your own initiative.
  • Provide outreach as part of home visits in clients' own environments.

As an Outreach Support Worker, you will provide high-quality, person-centred support to individuals with Learning Disabilities, Autism, Mental Health needs, and complex support requirements, helping them achieve their personal goals, develop life skills, and maintain meaningful connections within their community.

Where you’ll be working: Bromley, Bexley & Newham

You will provide outreach support to individuals within their own homes and local community.

We are looking for support workers to join our amazing team, and to continue delivering outstanding outcomes.

What is required from you?
  • A minimum of 12 months on your right to work
  • Full UK Drivers Licence

  • Supporting – Activities of daily Living including social activities, personal care, routines, planning and preparation of meals and budgeting.

  • Engaging - promoting and motivating people we support to enhance their quality of life
  • Specialising - supporting people who may need positive behavioural support approaches
  • Recording - keeping accurate records & reporting changes in health or circumstances of service users to management
  • Team player – Working as part of a team, as well as being able to work using your own initiative.
