Job summary

BHRUT in Romford is actively recruiting a Band 4 Discharge Coordinator to join the Hospital Complex Discharge Team. The role supports inpatient wards across Queen’s Hospital and King George Hospital to plan safe, timely discharges.

You will collaborate with patients, families and the MDT, identify delays early, maintain accurate records, and help meet NHS access targets; weekend work may be required to ensure smooth patient flow.

Qualifications

• Experience in patient-focused or social care environment.
• Experience working with multidisciplinary teams (MDT).
• Knowledge of discharge policy and pathways.
• Experience in a clinical setting.

Responsibilities

• Meet patients within 24 hours of admission to gather history for discharge planning.
• Identify potential discharge delays and record realistic Estimated Discharge Dates (EDD).
• Attend clinical board rounds to coordinate discharge actions.
• Identify discharges for the day and next day, aiming for same day where possible.
• Support and coordinate with wards, Discharge Leads and community partners.
