Job summary

HealthHero Integrated Care is recruiting independent, autonomous Prescribing Clinicians to support our Urgent Care service across Bath, Swindon and Wiltshire. Working from Chippenham Headquarters or Primary Care Centres, you will provide face-to-face care, home visits with a fully equipped medical car, and remote triage from the regional Head Office.

You will be a registered nurse, paramedic or allied health professional with a non-medical prescribing qualification and at least two years

Qualifications

• Autonomous, independent prescribing clinician with broad scope of practice.
• Two+ years prescribing experience in urgent care.
• MSc in Advanced Practice or equivalent.
• Experience in clinical assessment and triage across adults and paediatrics.

Responsibilities

• Autonomous clinical assessment, diagnosis, and treatment for adults/paediatric patients.
• Accurate triage and prioritisation using varied history taking.
• Undertaking home visits with a fully equipped medical car.
• Refer patients to alternative care settings where appropriate.
• Support Care Coordination from the regional Head Office.
• Triage NHS111 calls during core hours and assist other clinicians on shift.
