Job summary

HealthHero Integrated Care is recruiting an Advanced/Enhanced Clinical Practitioner to act as an autonomous prescribing clinician within Care Coordination and Integrated Urgent Care across Bath, Swindon and Wiltshire. The role supports NHS111 referrals, remote triage, and out-of-hours care, with shifts at our Chippenham HQ and Primary Care Centres.

You will assess, diagnose and prescribe where appropriate, coordinating with ambulance services and community teams to reduce admissions and improve

Qualifications

• Registered nurse, paramedic or allied health professional with non-medical prescribing qualification.
• MSc in Advanced Practice or equivalent experience.
• Minimum of 2 years prescribing experience.
• Completion of a recognised clinical reasoning module (e.g. PADRIP or equivalent).

Responsibilities

• Deliver safe clinical triage and prioritisation across ages.
• Triage and refer to appropriate care pathways to avoid unnecessary conveyance.
• Provide remote and face-to-face assessments, and home visits when needed.
• Support ambulance teams with real-time clinical decision support.
• Contribute to integrated, whole-system care delivery improvements.
