HealthHero Integrated Care in the Bath, Swindon and Wiltshire area seeks independent Prescribing Clinicians for urgent care out-of-hours. Roles include shifts at Chippenham HQ and/or Primary Care Centres with home visits, triage, and remote care.

As autonomous practitioners, you will assess, diagnose, treat, and refer patients, delivering timely, patient-centred care within a multidisciplinary team and across the broader urgent care pathway.

Competenze

• Autonomously provide clinical assessment, diagnosis, and treatment for adults/paediatric patients.
• Able to triage with varied techniques and assess past medical and drug history.
• Experience undertaking home visits with a fully equipped medical car.
• Refer patients to appropriate care settings when needed.
• Support Care Coordination and NHS111 triage during core hours.
• Demonstrate wide knowledge base and competency in clinical practice.
• Complete a clinical reasoning module (e.g. PADRIP or equivalent).
• Prescribing experience of at least 2 years.

Mansioni

• Autonomous clinical assessment and treatment at Primary Care Centres or remote consultations.
• Accurate triage and prioritisation of patients using diverse history-taking techniques.
• Undertake home visits to patients unable to travel, with a medical car and driver.
• Refer patients to alternative care settings where appropriate.
• Support with Care Coordination from regional Head Office.
• Triage NHS111 referrals during core daytime hours.
• Support other clinicians on shift as required.
