Job summary

Shrewsbury Primary Care Network's Home Visiting Service is recruiting a Paramedic Practitioner to perform home visits after GP triage. You will assess and manage patients with urgent conditions, with some clinic sessions, largely working solo while contributing to a structured development programme.

The role includes independent prescribing where qualified, DBS check, and CPD evidence. A comprehensive development plan with Level 7 study is provided and supported by network leadership.

Qualifications

• Paramedic Practitioner qualification evident.
• CPD evidence required.
• Level 7 portfolio or equivalent completed or willingness to undertake.

Responsibilities

• Conduct home-based clinical assessments and management for triaged patients.
• Diagnose, treat or signpost undifferentiated conditions during home visits.
• Refer to Care Co-ordination Centre and other pathways; admit to secondary care when needed.
• Prescribe for common urgent conditions within personal formulary if independently qualified.
• Document all patient contacts accurately per policy.
• Mentor and supervise junior colleagues and peers.
