Job summary

Kent and Medway NHS and Social Care Partnership Trust is seeking an Advanced Nurse Practitioner who will work autonomously within their professional scope, providing face-to-face care, telephone consultations, triage and home visits as appropriate.

The post-holder will manage patient caseloads, arrange investigations and referrals, interpret results, and deliver programmes of supported self-management to improve activity, mobility and independence while minimising pharmacological intervention.

Qualifications

• Qualified Advanced Nurse Practitioner with current registration.
• Experience in autonomous practice.
• Experience in triage and patient management.

Responsibilities

• Provide face-to-face and telephone consultations; triage and manage patient needs.
• Prioritise and triage patient cases; determine referrals for investigations and admissions.
• Manage patient caseloads; interpret results and coordinate care with colleagues.
