Purosearch Ltd is assisting a welcoming GP practice in Maidstone, Kent, in recruitment for an Advanced Nurse Practitioner / Advanced Clinical Practitioner / Paramedic Practitioner. This role offers autonomy in a supportive team within primary care.

You will manage a varied caseload, including acute and chronic conditions, with a mix of face-to-face, triage, and home visits. Independent prescribing would be advantageous and development opportunities are available.

Qualifications

• Advanced clinical practice qualifications and experience.
• Experience working autonomously within primary care.
• Competent in assessing acute presentations.
• Experience managing long-term and complex conditions.
• Strong clinical reasoning and decision-making.
• Ability to work independently and as part of a multidisciplinary team.
• Registration and qualifications for your clinical background.
• Independent prescribing would be advantageous.

Responsabilités

• Independently assess, diagnose and manage patients presenting with acute and minor illnesses.
• Manage patients with long-term conditions, complex needs and multiple co-morbidities.
• Undertake face-to-face and telephone consultations, triage and home visits where required.
• Request and interpret appropriate investigations, including blood tests and imaging.
• Prescribe and review medication within your professional scope of practice.
• Develop appropriate treatment plans and make referrals to primary and secondary care services.
• Provide patients with advice on self-management, prevention and health improvement.
• Work closely with GPs, Practice Nurses and the wider multidisciplinary team.
• Maintain accurate clinical records and deliver care in line with NICE guidance and evidence-based practice.
• Contribute to clinical development, service improvement and the sharing of knowledge across the team.
