Autonomous Clinician: Triage & Home Visits

Job summary

NHS Jobs is seeking a clinically focused practitioner to undertake home visits as assessed and when appropriate, assess and triage acute presentations (including telephone triage), and contribute to a minor illness/injury walk-in service.

You will assist in training frontline staff, make direct referrals within locally agreed pathways, interpret ECG and ABPM results, and ensure complete and accurate documentation of every patient contact.

Responsibilities

• Undertake home visits as assessed and when appropriate
• Assessment and triage of acute presentations including telephone triage
• Contributing towards the running a minor illness/ injury walk-in service
• Assisting in the training of frontline staff
• Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols
• ECG and ABPM interpretation
• Assisting with other ad hoc clinical tasks
• Ensure complete and accurate documentation of every patient contact
• Undertake the collection of pathological specimens including intravenous blood samples, swabs etc.
• Perform investigatory procedures requested by the GPs
