Job summary

• Deliver clinical care, treatment, and interventions, including medications where appropriate.
• Manage admission and discharge of patients from the caseload.
• Complete continuing healthcare checklists and support fast-track packages of care from external agencies.
• Assess mental capacity and work within legal frameworks where needed.
• Monitor and improve standards of patient care while safeguarding privacy and dignity.
