Central London Community Health Trust (CLCH) Hertfordshire Adult Community Services is recruiting for a Band 5 nurse to join the West Herts community nursing team. The role focuses on providing skilled community nursing care to housebound patients, coordinating care and delivering person-centred support to enable home-based living and, where possible, prevent unnecessary hospital admissions.

You will work under supervision, develop new pathways, and participate in learning opportunities through

Qualifications

Registered Nurse with full and current UK NMC registration
Evidence of post-registration education and training
Recent clinical nursing experience in multidisciplinary teams
Experience in caring for patients with complex health needs
Experience in care planning, documentation and nursing process

Responsabilités

Provide skilled nursing care including chronic disease management, end of life care, wound management and catheterisation where required.
Act as Named Nurse under supervision, manage holistic health assessments and care planning.
Undertake comprehensive assessments of physical, psychological and social care needs using the Single Assessment Process.
Promote self-care and coordinate patient care to prevent unnecessary hospital admissions.
Advocate for patients and support informed choices, independence and self-care.
Maintain accurate records in line with Trust and NMC guidelines.
