Job summary

Lincolnshire Community Heath Service (LCHS) is looking for a Registered Nurse to join the Lincoln North Community Nursing Team. The role involves autonomous practice and teamwork to deliver nursing care in patients' homes, care homes and clinic settings.

You will assess care needs, plan and evaluate care using evidence-based practice and work closely with the MDT to coordinate patient care and improve outcomes.

Qualifications

• Assess care needs and plan, deliver and evaluate nursing care in clients' homes, care homes and clinics.
• Work autonomously and as part of the MDT with social workers, GPs and allied health professionals.
• Provide wound management, continence care, palliative care and medication administration.
