Job summary

Livewell Southwest in Plymouth is recruiting a Community Registered Nurse to coordinate care for housebound patients home visits and care homes. You will work within a multidisciplinary team, applying evidence-based practice and maintaining high professional standards.

Qualifications

• Registered nurse with current NMC registration.
• Experience in community nursing and home care.
• Ability to travel frequently between sites and community locations.
• Excellent communication and teamwork.

Responsibilities

• Coordinate care for housebound patients using evidence-based practice.
• Assess, triage, plan, implement and evaluate care.
• Support team as autonomous practitioner.
