Job Overview

The Community Learning Disability Nursing Team supports people with learning disabilities to improve and maintain their health, promote independence, reduce health inequalities, and improve wellbeing. The team also works closely with families, carers, and partner agencies to provide holistic, person-centred care. As a Band 6 Community Learning Disability Nurse, you will manage a caseload, provide clinical leadership, and deliver high-quality, evidence-based care to service users with varying and complex needs.

Key Responsibilities Include
  • Managing and coordinating care for a designated caseload.
  • Assessing, planning, implementing, and evaluating care programmes.
  • Promoting patient safety, positive outcomes, and patient experience.
  • Providing clinical advice, support, and professional leadership.
  • Contributing to service development and quality improvement.
  • Supporting, supervising, and mentoring students and colleagues.
  • Collaborating with multidisciplinary teams and key stakeholders.
  • Developing effective partnerships with internal and external services.
  • Supporting individuals, families, and carers to achieve positive health outcomes.

A valid UK driving licence and access to a vehicle insured for business use are essential, as travel across community settings is required.

Job Description Summary (Main Duties And Responsibilities)
  • Co-ordinate, prioritises, facilitate and monitor the delivery of care for patients on a designated caseload and to other patients as deemed appropriate.
  • Function asa professional and clinical role model to all staff, patients, carers, and the public always.
  • Supervise the performance standards of own team members where appropriate.
  • Maximise use of clinical and staff resources, implementing best practice in patient pathways, admission, and discharge arrangements.
  • Plan and promote the development of new services within the designated area.
  • Maintain efficient and effective communication on issues related to patient care, caseload, and organisation management.
  • Participate in staff recruitment and selection processes and wider recruitment and retention strategies.
  • Act in a professional manner and ensure self and colleagues always adhere to the LPT Uniform Policy.
