Job summary

Parish Fields Practice in Diss is seeking an enthusiastic Advanced Nurse Practitioner to join a multidisciplinary team. You will assess and manage patients with acute, undifferentiated and long-term conditions, working autonomously within your scope.

The role offers proactive collaboration with GPs and the MDT, supporting clinical development, education and service improvement while delivering high-quality patient care.

Qualifications

• Registration with the NMC is essential.
• Independent prescribing qualification (V300 or equivalent) required.
• A masters degree level qualification in a relevant area is essential.
• Evidence of working at masters level across clinical practice, leadership and education.

Responsibilities

• Assess and manage patients presenting to the practice, including acute and long-term conditions.
• Work autonomously within your scope of practice using advanced clinical skills.
• Assess, diagnose, treat and refer patients appropriately.
• Prescribe safely within your scope and UK guidelines.
• Collaborate with GPs and the wider MDT team.
• Support personalised care and shared decision-making.
• Contribute to clinical development, quality improvement and service development.
• Support the education and supervision of colleagues.
