Job summary

NCPC (North Cumbria Primary Care) is seeking an experienced Advanced Clinical Practitioner to join the Lowther Medical Centre team. You will assess, diagnose, treat and manage patients autonomously, collaborating with GPs and the multidisciplinary team to deliver safe, effective primary care.

Qualifications

• Masters degree in advanced clinical required.
• Active NMC registration as a nurse.
• Non-medical prescriber status is desirable.
• Ability to work autonomously within a multidisciplinary team.
• Strong communication and interpersonal skills.
• Commitment to ongoing professional development.
• Experience in primary care settings is advantageous.

Responsibilities

• Making professionally autonomous decisions.
• Managing patients with undifferentiated and undiagnosed problems.
• Provide comprehensive healthcare sensitive to patients' needs, including assessment, diagnosing, prescribing, and referrals where appropriate.
• Conducting physical assessments to diagnose patients' conditions and provide self-care advice.
• Direct and participate in provision of continuing care, health maintenance, screening and health promotion.
• Managing and participating in Chronic disease management.
• Supporting and developing work towards agreed GMS targets.
• Any other duties to provide quality health care to patients.
