Job summary

Coleridge Medical Centre, part of The Fuller and Forbes Partnership, seeks an Advanced Nurse Practitioner to join for up to 24 hours per week, delivering patient-centred care in a busy general practice.

You will assess, diagnose, treat, prescribe and provide expert advice, with autonomous decision-making, following NICE guidance and national standards, while contributing to a frailty pathway to improve outcomes for older patients.

Qualifications

• Fully qualified Advanced Nurse Practitioner with independent prescribing.
• Experience in primary care and patient-centred care.
• Understanding and applying NICE guidance and national standards.

Responsibilities

• Deliver undifferentiated patient care during morning and afternoon sessions.
• Provide timely follow-up of patients and manage ongoing cases.
• Prescribe medications within scope of practice and current legislation.
• Lead on frailty pathway development to improve outcomes.
