Job summary

Leaders In Care Ltd is seeking a Registered Nurse to take on the Nurse Case Manager role supporting autistic adults across Manchester's supported living and community services. The role offers flexible part-time hours (15 hours over two days) with hybrid working and a Manchester-based caseload.

You will conduct clinical assessments, develop care plans, provide specialist input, ensure safeguarding and risk management, and collaborate with families and multidisciplinary teams.

Qualifications

• Current NMC registration
• Experience with autism
• Confident assessment and care planning skills
• Good safeguarding and clinical risk knowledge
• Strong communication and multidisciplinary working skills
• Ability to work autonomously across community services

Responsibilities

• Complete comprehensive clinical assessments and reviews
• Develop and update person-centred care plans and risk assessments
• Provide specialist nursing input for autistic service users
• Support individuals with complex health, behavioural and communication needs
• Provide clinical guidance and support to care teams
• Work closely with families, carers and multidisciplinary professionals
• Support safeguarding, clinical governance and risk management
