Permanent contract | £27,711 to £32,654 per annum (Scale F/G) | 37 hours per week | Attleborough Enterprise & Community Centre

We are seeking an enthusiastic community Assistant Practitioner to join our highly motivated, innovative team of Social Workers, Occupational Therapists, and Assistant Practitioners. This is an exciting opportunity to be part of a dedicated team supporting people to be independent, resilient, and well. We work in a multi-disciplinary environment, building strong partnerships with service users and their families, and are focused on achieving positive outcomes that are cost-effective and fit for purpose.

The team is experienced with complex cases and knowledgeable about the Care Act (2014), Mental Capacity Act (2005), and Safeguarding, with excellent time management and IT skills. New staff are supported to develop a thorough understanding of Norfolk County Council processes and procedures.

Covering South Norfolk, our team assesses and supports adults in the community using the Care Act (2014), via video, telephone, and face-to-face interactions. The role involves undertaking assessments, service commissioning, and re-assessment work. You should have excellent communication, record-keeping, and keyboard skills, experience working with vulnerable people, and a commitment to delivering excellent customer service, with familiarity with the Department's eligibility criteria.

We are looking to appoint either a Supported Assistant Practitioner (Scale F) or an experienced Assistant Practitioner (Scale G). Supported Assistant Practitioners will participate in a 9-month development programme, with the opportunity to progress to an Assistant Practitioner role upon successful completion.
