We are seeking an enthusiastic community Assistant Practitioner to join our highly motivated, innovative team of Social Workers, Occupational Therapists, and Assistant Practitioners. We pride ourselves on a multi-disciplinary environment and positive relationships with health colleagues. Our team is experienced with complex cases, knowledgeable about the Care Act (2014), Mental Capacity Act (2005), Safeguarding, and possesses excellent time management and IT skills. New staff will receive support to develop a thorough understanding of Norfolk County Council processes and procedures.

Covering South Norfolk, our team assesses and supports adults in the community using the Care Act (2014). We are investing in people who can help us advance our vision. As an Assistant Practitioner, you will undertake assessments, service commissioning, and re-assessment work via video, telephone, and face-to-face interactions. Strong communication, record-keeping, and keyboard skills are essential, along with experience working with vulnerable populations. A commitment to excellent customer service and familiarity with the Department’s eligibility criteria are also required.

Application Details

We are hiring for either a Supported Assistant Practitioner (Scale F) or an experienced Assistant Practitioner (Scale G). Supported Assistant Practitioners will participate in a 9-month development program, with potential to progress to an Assistant Practitioner role upon successful completion.
