The Social Care Community Engagement Team is leading Norfolk's 'Living Well' initiative, which aims to implement a new social care model focusing on strengthening community connections and meaningful conversations with individuals needing care and support. This approach emphasizes genuine interactions, local networks, and community support to improve care quality.
The team currently works in a hybrid model (home and office). Flexibility and adaptability to service demands are expected and will be reviewed.

We are seeking applications for either a Supported Assistant Practitioner (Scale F) or an experienced Assistant Practitioner (Scale G).
