Job description

Job Title: Social Care Community Engagement Team Member

Contract Type: Permanent | Salary: £27,711 to £32,654 per annum (Scale F/G) | Hours: 37 hours per week | Location: County Hall, Norwich

About the Role

The Social Care Community Engagement Team is leading Norfolk's 'Living Well' initiative, which aims to implement a new social care model focusing on strengthening community connections and meaningful conversations with individuals needing care and support. This approach emphasizes genuine interactions, local networks, and community support to improve care quality.

Key Responsibilities
  • Engage with individuals to understand their needs and promote personalised care.
  • Maintain accurate records and manage a high volume of work efficiently.
  • Collaborate with multidisciplinary teams and adhere to eligibility criteria.
  • Participate in supervision, peer support, and training to develop skills.
Candidate Requirements
  • Excellent communication, record-keeping, and keyboard skills.
  • Experience working with individuals requiring care and support.
  • Strong customer service orientation and organizational skills.
  • Commitment to multidisciplinary working and understanding of eligibility criteria.
Work Arrangement

The team currently works in a hybrid model (home and office). Flexibility and adaptability to service demands are expected and will be reviewed.
