Zusammenfassung

Kent and Medway Mental Health NHS Trust is seeking a discharge co-ordinator to support the early and complex discharge process across Younger and Older Adult services. You will help ensure smooth patient journeys, assist with bed management via rota-based bed requests, and work with multidisciplinary teams to remove discharge barriers.

The role involves liaising with internal teams and external agencies, assisting patients and carers with housing options, and travelling to other hospital sites

Qualifikationen

• Experience in mental health
• Experience of working in a community or hospital setting
• Good knowledge of acute inpatient admission and discharge processes
• Knowledge of the Care Act and its implications for practice
• Clear verbal and written communication
• Computer literacy and keeping records

• Work with the MDT to identify discharge barriers early in the patient journey

• Represent the Patient Flow team in individual patient review meetings
• Contribute to Patient Flow team discussions and discharge planning
• Travel to non-trust hospital sites

This nine-month post sits in the Discharge team at St Martin's Hospital, supporting both Younger and Older Adult services at Kent and Medway Mental Health NHS Trust. The work centres on coordinating early and complex discharges so patients move on smoothly and bed capacity is used well. It also involves working with the multidisciplinary team and outside agencies to remove barriers to discharge, taking bed requests on a rota, and travelling to other hospital sites. A full UK driving licence and access to a car for business use are required, unless a disability exempts the post holder under the Equality Act.
