Job summary

Rutland County Council is seeking an Assistant Care Manager to support adults and carers in a strength-based, person-centred approach. You will triage referrals, assess eligibility under the Care Act, and develop support plans in collaboration with health and partner agencies.

The role requires strong communication and organisational skills, a clear understanding of adult social care legislation, and a proactive, compassionate mindset.

• Good understanding of adult social care legislation, safeguarding responsibilities, and Care Act processes.

Responsibilities

• Work in a team taking referrals, triaging, and identifying eligibility for Adult Social Care.
• Provide information, advice and guidance for referrals that need signposting to other partners and universal services.
• Undertaking strengths-based, person-centered and outcome-focused Care Act needs assessments, support planning and proportionate short-term interventions.
• Identify risk and seek support where complexity requires and escalate to the senior practitioner.
• Liaise with the wider team and other Independent and Voluntary Agencies.
• Update and maintain case file notes and electronic records.
• Work in an integrated way with health and partners across LLR and locally.
