Job summary

Rutland County Council is seeking an Assistant Care Manager to work within Adult Social Care. You will work in a team, triaging referrals, identifying eligibility, and supporting planning to deliver outcomes for adults with care needs.

The role emphasizes strengths-based, person-centred approaches, safeguarding and partnership working with health and local agencies. Driving licence is required to travel across the county.

Qualifications

• Experience in adult social care legislation and safeguarding responsibilities is desirable.
• Strong communication and organisational skills are essential.
• Ability to build effective relationships with service users, carers and partner agencies.

Responsibilities

• Work in a team taking referrals, triaging and identifying eligibility for Adult Social Care.
• Provide information, guidance and signposting for referrals to universal services.
• Undertake strengths-based, person-centred care act needs assessments and support planning.
• Maintain case notes and electronic records; liaise with health colleagues and partners.
• Travel across the county with a full driving licence.
