Job summary

Belvoir Health Group is seeking a Registered Nurse to join our Practice Nursing team across three surgeries in Cropwell Bishop, Bingham and Cotgrave. The role focuses on long-term condition management, immunisations, wound care and health promotion, aiming to deliver high-quality, patient-centred care.

You will work with Senior Nurses and GP teams, perform venepuncture, ECGs, travel health assessments, and contribute to care plans and recall programmes.

Qualifications

  • NMC registered and eligible to practice in the UK.
  • 1 year post qualifying experience is essential.
  • Experience in General Practice is welcome but not essential.

Responsibilities

  • Provide treatment room nursing care including wound management, venepuncture, ECG, injections.
  • Discharge or refer patients as appropriate.
  • Provide triage service by telephone per protocols.
  • Organise vaccination and immunisation activities and health screening.
  • Maintain accurate records and recall systems for chronic disease patients.
  • Supervise Health Care Assistant, phlebotomists and nursing students.

The successful candidate will work across our three surgeries located in Cropwell Bishop, Bingham and Cotgrave. Belvoir Health Group provides care for a diverse patient population of over 26,000 patients. You will play a key role in delivering high-quality, patient-centred Practice Nursing services, supporting patients to improve and maintain their health and wellbeing. The role includes:

  • Supporting the management of long-term conditions
  • Health promotion, prevention and screening
  • Wound care
  • Immunisations and vaccinations

  • Developing, implementing and evaluating care plans
  • Identifying and supporting patients at risk of developing long-term conditions

This is an excellent opportunity to develop your clinical skills within a supportive and experienced team environment., To work as a Practice Nurse within the organisation taking responsibility for assessing the nursing needs of patients attending surgery with a broad range of long term All duties and responsibilities to be carried out in accordance with protocols developed by the Nursing Team and in agreement with the Nurse Clinical lead. To provide a high quality, evidence based easily accessible service for patients. To be able to time manage during clinics to ensure a seamless service that is delivered on time, as far is reasonably practical, to the patients. To work with the Senior Nurses in liaisons with the site Team Leaders on a day to day basis and particularly with regard to arrangements for patient recall and review, holidays and rotas, and as a member of the surgery team. MAIN DUTIES AND RESPONSIBILITIES To provide treatment room nursing care. To include wound management, venepuncture, ear irrigation, ECG, routine injection and urinalysis. To assess and treat minor illness and injury in accordance with protocols. To discharge or refer patients as appropriate. To provide a triage service for patients, including by telephone, offering advice if appropriate in accordance with protocols. To organise medication and wound dressings with a GP/Non Medical Prescriber as part of a plan of care for an individual patient as needed. To assess those patients wishing to travel. To offer basic vaccination, malaria prophylaxis and advice about where to seek more specialist advice. To provide contraceptive, sexual health, coil check and menopause advice and administer depo-provera injections according to protocol. To ensure patients are monitored appropriately depending on their chosen method of contraception or menopause treatment. To provide a cervical screening service in accordance with the National Policy and local NHS recall system. To be able to carry out routine swabs as part of a request by GP/ANP as required. To provide childhood and adult immunisations in accordance with the British Vaccination Schedule. To provide screening examinations e.g. new patient health check, elderly screening and lifestyle advice as requested by GP colleagues or as part of a recall system. To visit patients in their homes to undertake work on behalf of the Practice as requested by the GP/Practice Nurse Manager. To assist in the provision of care to those with long term conditions from the list below, according to skills, training and experience - To take responsibility for maintaining the register and recall of patients with coronary heart disease within the surgery. To share the workload and responsibility with GPs for the care of these patients in accordance with the protocol. To take responsibility for maintaining the register and recall of patients with asthma within the surgery. To share the workload and responsibility with GPs for the care of these patients in accordance with the protocol. To take responsibility for maintaining the register and recall of patients with COPD within the surgery. To perform spirometry. To analyse and inform the GP of the results. To share the workload and responsibility with GPs for the care of these patients in accordance with the protocol. To take responsibility for maintaining the register and recall of patients with diabetes within the surgery. To share the workload and responsibility with GPs for the care of these patients in accordance with the protocol. To share the workload and responsibility with GPs for patients with hypertension in accordance with the protocol. In cases of co-morbidity, to work closely with the lead GP and Practice Nurse Manager, as appropriate in accordance with protocol and personal competencies. Aside from any provision of chronic disease management care, to perform general medication reviews with patients. To be responsible for the maintenance of accurate records, reflecting consultations and communications with patients. To ensure the careful labelling of specimens and printing of prescriptions. To ensure nursing care is evidence based and evaluated appropriately. To undertake a role in research and audit, independently or in conjunction with other members of the team. To act as a representative for Belvoir Health Group at CCG level, as delegated by Practice Nurse Manager To take responsibility for the teaching and supervising of the Health Care Assistant, phlebotomists and Nursing students as required. To attend designated meetings and relevant training courses keeping professionally up-to-date. To be fully conversant with the in house complaints procedure. To be responsible for ensuring that patient queries and complaints are dealt with according to the procedures laid down by the Practice, adhering to the NHS Executive guidelines and requirements. To make sure that the clinical room is kept tidy, and instruments are sterilised as required. To participate actively in Infection Control and Health and Safety audit. Any other duties within the range and scope of this Job Specification as deemed necessary and appropriate to skill by the Practice Nurse Manager as the organisation and structure of the Nursing Team moves forward.
