Boston Primary Care Network is seeking an experienced Advanced Clinical Practitioner to join our Home Visiting Wellbeing Team, covering Boston area. The role focuses on urgent on-the-day home visits, triage and managing acute and minor illnesses for housebound patients and care home residents.

You will be part of a supportive, multi-disciplinary team, employed by Boston PCN Ltd, working closely with GP practices to deliver patient-centered care in the community.

Competenze

• Advanced history-taking, physical assessment and clinical examination skills.
• Recognise and respond to red flags, clinical risk and deterioration.
• Safe prescribing and medicines management knowledge.
• Knowledge of safeguarding, consent, capacity, confidentiality and information governance.
• Excellent communication, consultation and shared decision-making skills.
• Ability to prioritise workload, manage uncertainty and work effectively under pressure.
• Competent use of clinical IT systems, ideally including SystmOne.

Mansioni

• Carry out urgent on the day home visits, including triage.
• See patients for acute and minor illness, including housebound and care home residents.
• Work on behalf of Boston PCN practices.
• Be employed and supported by Boston PCN Ltd and collaborate with a diverse, multi-disciplinary team.
