Practice Plus Group is recruiting an Advanced Nurse / Clinical Practitioner to join our Clinical Assessment Service in a remote capacity. Work in a flexible, home-based role delivering timely clinical advice and guiding patients to the right care pathway across the region.

You will be part of a dynamic, multidisciplinary team, using telephone and digital triage to ensure patients receive care in the right place and at the right time. Training and development are provided.

Qualifications

• Part of a multidisciplinary team delivering care.
• Role involves clinical decision support and pathway navigation.
