Job summary

Derbyshire Community Health Services NHS Foundation Trust is recruiting an Enhanced Clinical Practitioner in Derby City Community Service, up to 37.5 hours per week, covering home visiting, urgent community response and anticipatory care. The post will provide medical assessment and management, with mentoring and support for clinical decision making, and development of new pathways with the team.

Candidates may be appointed at Band 6 or 7 depending on experience; trainees may start at Band 6

Qualifications

• ECP/trainee with clinical skills and a willingness to work in community settings.
• Experience in medical assessment and management is desirable but not essential for trainees.

Responsibilities

• Provide medical assessment and management for home visiting and complex care teams.
• Mentor and support MDT members to aid clinical decision making.
• Develop new clinical pathways and skills in line with local standards.
