Job summary

Chesterfield Royal Hospital NHS Foundation Trust is seeking a fully qualified Advanced Clinical Practitioner to join our acute frailty team in Frailty & Geriatric medicine. You will deliver early Comprehensive Geriatric Assessment and provide leadership and education to the team to drive service improvements.

The post involves working with Acute Frailty consultants, making advanced clinical decisions, and contributing to policy development while maintaining high standards of patient care for

Qualifications

Registered healthcare practitioner.
ALS provider (or willing to complete).
MSc in Advanced Practice within 0–1 year.
Masters in Advanced clinical practice.
ALS instructor.
Independent prescriber.
Ability to interpret CT brain and Skeletal X-rays.

Responsibilities

Provide high quality clinical assessment, intervention and treatment for frail older people.
Deliver early Comprehensive Geriatric Assessment within the Acute Frailty teams.
Provide clinical leadership and education to support service development.
Work with Acute Frailty consultants and other teams to ensure high standards of care.
Support junior staff and contribute to policies and governance.
