Job Summary

Job Responsibilities

  • Deliver holistic care using expert knowledge and clinical skills.
  • Assess, diagnose, and treat patients with acute, non-acute, and chronic conditions.
  • Refer patients to specialists, multidisciplinary team members, secondary care, and voluntary organizations as appropriate.
  • Perform and interpret diagnostic tests and routine procedures.
  • Make independent decisions where precedents do not exist, including direct referrals to hospital consultants.
  • Support and advise colleagues when protocols do not apply.
  • Communicate sensitive condition-related information effectively.
  • Initiate diagnosis processes for chronic diseases and refer to appropriate staff.
  • Contribute to practice targets like QOF, prescribing schemes, and benchmarking.
  • Audit care outcomes and implement improvements.
  • Work within professional guidelines and practice policies.
  • Possess extended and supplementary prescribing qualifications.
