I am excited to announce a recruitment opportunity for a Band 7 medical prescriber/Advanced Clinical Practitioner to join the team. Our client provides NHS111 and Out of Hours GP services to over 4.5 million patients across Hertfordshire, Cambridgeshire, Bedfordshire, West Essex, and the South West of England.

The Advanced Clinical Practitioner plays a pivotal role in delivering high-quality, patient-centered care in a dynamic healthcare environment. You will assess, diagnose, and manage a wide range of acute and chronic conditions, integrating evidence-based treatments and holistic patient care. Working autonomously and collaboratively within a multidisciplinary team, you will contribute to improving patient outcomes through expert clinical decision-making, diagnostic skills, and management of long-term health conditions.

This role requires a strong commitment to patient advocacy, health promotion, and preventive care. You will perform complex assessments, prescribe treatments, and guide medication management, vaccinations, and lifestyle interventions. Supporting patients in self-care, mental health, and urgent situations ensures seamless, compassionate healthcare delivery.

Your new role
  • Assess, diagnose, plan, implement, and evaluate treatment for patients with acute and chronic conditions, adopting a holistic approach.
  • Perform physiological and psychological assessments, prioritizing urgent and complex cases, including emergency care.
  • Prescribe, review, and advise on medications, ensuring safety and effectiveness per guidelines.
  • Support health promotion, vaccination programs, family planning, sexual health, and wound care services.
  • Identify, assess, and refer mental health concerns for appropriate care.
  • Collaborate within a multidisciplinary team to provide integrated, patient-focused care, contributing to clinical governance and service improvements.
What you'll need to succeed

Essential criteria

  • NMC Registered Nurse or HCPC Registered Paramedic
  • MSc in Advanced Clinical Practice or equivalent
  • Minor illness and Minor injury qualifications (accredited)
  • Prescribing certificate
  • Triage/streaming experience
  • Experience with a variety of patients with acute and chronic conditions of all ages
  • Leadership experience
  • Knowledge of the four pillars of advanced practice
  • Experience managing end-of-life care, including prescribing/administering medications like 'Just in Case'
  • Experience in Family Planning
  • Experience managing chronic conditions
  • Experience working in Emergency Department, UTC, or GP settings
  • Audit or research experience
  • Experience reviewing X-rays
