Job summary

Advanced Clinical Practitioner - Band 7

£51,883 - £58,544 Inc HCAS per annum pro rata

Full-time - 37.5 hours per week

We are looking for a proactive Advanced Clinical Practitioner who wants to share their skills within our team.

The post is based within the Urgent Community Response Team. This service provides a multi-disciplinary urgent assessment and crisis intervention service within a 2-hour response, and at times a 2-4 hour response to patients in the community with the aim of preventing hospital admission and supporting early discharge from hospital. It operates 7 days a week from 8 am to 8 pm.

The post holder is an Advanced Clinical Practitioner, acting within their professional boundaries. They will be working towards becoming a non-medical prescriber with good community experience, able to see patients independently and make autonomous decisions in clinical and diagnostic assessments of patients with potentially complex health and social care needs to support keeping them in the community and reduce hospital admissions and A&E attendance. They will develop a comprehensive plan of care.

Applicants MUST hold professional registration with NMC or HCPC; applications without this will not be considered.

Main duties of the job
  1. Respond to urgent referrals from GPs, Acute Trust, 111, St Christopher's hospice, Ambulatory Services, and community providers for patients to be seen at home. Manage deteriorations due to illness or injury, prescribe medications, and develop care plans to prevent A&E attendance or hospital admission. Support early discharge with symptom monitoring.
  2. Develop high-level skills in clinical assessment and diagnostic reasoning to manage high-acuity users with multiple co-morbidities, aiming to reduce hospital admissions.
  3. Support partnership working with GPs, ANP/ACP/Clinical lead, nursing home staff, and other healthcare professionals to promote anticipatory care planning, supporting the unscheduled care agenda.
  4. Maintain knowledge of evidence-based practice and clinical management options, demonstrating expertise and clinical reasoning.
  5. Ensure personal performance aligns with job requirements, professional standards, and organizational policies.
  6. Complete mandatory training and ongoing professional development.
  7. Maintain a portfolio of advanced practice activities and evidence.
  8. Participate in clinical supervision and appraisal processes.
  9. Identify own learning needs and plan strategies for personal development.

Person Specification
Qualifications (Essential)
  • Undergraduate degree in a suitable discipline
  • Registration with NMC or HCPC
  • Registered non-medical prescriber (if appropriate)
  • Completed advanced clinical assessment or ECAS module
  • Autonomous practice and decision-making skills
  • Willingness to work towards full ACP MSc, portfolio, or credentialing
  • Evidence of Continuing Professional Development
Qualifications (Desirable)
  • Master's level award covering all four pillars of advanced practice or credentialing equivalent
  • Working towards or planning to complete non-medical prescribing
