Job Summary
Working with the ADHD service, the Advanced Clinical Practitioner (ACP) manages the complete clinical care for their patients, using expert knowledge and clinical competence to support multi-professional teams within Sheffield Health and Social Care. The ACP ensures high standards of clinical care in accordance with the Nursing and Midwifery Council and Health and Safety Legislation.
The ACP acts autonomously in assessment, diagnosis, treatment, including prescribing, of patients with complex problems and can work across all clinical settings. They work across boundaries, providing comprehensive clinical care as part of a multidisciplinary team.
Responsibilities include assessing, diagnosing, and managing patient needs, delivering evidence-based care, providing clinical leadership, and advising team members. The ACP is also an Independent Non-Medical Prescriber with extensive prescribing experience.
Main Duties
- Provide clinical leadership for complex cases, leading directly or with the MDT.
- Offer expert clinical intervention using their knowledge, skills, and behaviours.
- Navigate organizational boundaries to optimize outcomes for service users, families, and carers.
- Work autonomously within ethical and legal frameworks, responsible for decisions and actions.
- Conduct systematic history taking and clinical examinations.
- Utilize clinical reasoning for differential diagnosis and person-centred management plans.
- Manage uncertainties, comorbidities, and recognize when specialist opinions are needed.
- Demonstrate advanced technical skills and knowledge as per MSc ACP curriculum.
- Support service transition and ensure patient safety and risk mitigation.
