Cardiff and Vale University Health Board is recruiting Advanced Clinical Practitioners to join our bank workforce for Out Of Hours shifts. You will assess, diagnose, plan, implement and evaluate care for patients with undifferentiated or complex needs, on-site in our 24/7 service after 6pm weekdays and on weekends.
Candidates must hold full NMC registration; MSc in Advanced Clinical Practice is desirable; expect autonomous working, experience in urgent care, and Non-Medical Prescriber V300.
Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) are seeking highly skilled and motivated ACPs, ANPs & APNPs to join our bank workforce with opportunities within Out Of Hours. We are recruiting for our bank, where we offer the flexibility to fit around you., Out-of-hours (OOH): Onsite 24/7 service - Weekdays after 6pm and Weekends Assess, diagnose, plan, implement and evaluate treatment interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national local protocols and within scope of practice. Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.
