Job summary

Service Care Solutions is seeking an Advanced Clinical Practitioner to join our on-site team across Greater Manchester. The role covers clinical, leadership, education and research, with flexible, ongoing shifts and a pay rate of £40–£55 per hour (Ltd).

You will independently assess, diagnose, manage and prescribe within scope, provide triage for complex cases, and collaborate with stakeholders to maintain high-quality, patient-centred care. Travel and out-of-hours work may be required.

Qualifications

• Hold an MSc in Advanced Clinical Practice (ACP) and NMP qualification.
• Practice across four pillars: clinical, leadership, education and research.
• Independently assess, diagnose, manage and prescribe within scope.
• Provide expert care for undifferentiated, complex, urgent or minor illness.
• Undertake comprehensive history taking, clinical examination and investigations.
• Develop, implement and evaluate safe, evidence-based plans.
• Prescribe safely within scope and comply with prescribing governance.
• Make referrals, transfers and discharges with safety-netting.
• Provide telephone and clinical triage across services.
• Communicate with patients, colleagues and external stakeholders.
• Maintain accurate records and use relevant IT systems.
• Work in accordance with professional standards.
• Identify clinical risk, safeguarding, infection control and health & safety.
• Participate in supervision, revalidation, audit and learning.
• Collaborate as senior member of clinical team to support service delivery.
• Demonstrate flexibility including travel and out-of-hours work.

Role Overview

• Practice across the four pillars of advanced practice: clinical, leadership, education and research.
• Independently assess, diagnose, manage and prescribe within professional scope and competence.
• Provide expert care for patients presenting with undifferentiated, complex, urgent or minor illness.
• Undertake comprehensive history taking, clinical examination and appropriate investigations.
• Develop, implement and evaluate safe, evidence-based treatment and management plans.
• Prescribe safely within scope of practice and comply with local, national and organisational prescribing governance.
• Make appropriate referrals, transfers and discharges, including effective safety-netting and follow-up.
• Provide effective telephone and clinical triage across a range of services.
• Communicate sensitively and effectively with patients, colleagues and external stakeholders.
• Maintain accurate, contemporaneous clinical records and use relevant technology and information systems.
• Work in accordance with professional standards
• Identify and manage clinical risk, safeguarding concerns, infection control and health & safety requirements.
• Participate in clinical supervision, professional development, revalidation, audit and shared learning.
• Work collaboratively as a senior member of the clinical team, supporting effective workforce and service delivery.
• Demonstrate flexibility to meet service needs, including travel and in/out-of-hours working, while upholding the organisation’s values of patient-centred, high-quality and innovative care.
