Job summary

Integrated Care 24 (IC24) is seeking an Advanced Clinical Practitioner to join the East Sussex Home Visiting Service in Brighton and surrounding areas. You will conduct high-quality clinical assessments and provide treatment and prescribing for patients in their homes, supporting urgent and primary care services.

You will work autonomously within your professional scope, making safe decisions, reducing hospital admissions, and contributing to service improvement.

Qualifications

• Professional registration with the NMC, HCPC or GPhC.
• Independent Prescriber qualification.
• Advanced assessment and minor illness qualification (Level 6 or 7).
• Proven experience of autonomous clinical practice.

Advanced Clinical Practitioner (ACP) – Home Visiting Service

The Role

Advanced practice is where clinical expertise, autonomy and decision-making come together.

It’s where you assess complex presentations, manage uncertainty, and make timely decisions that have a direct impact on patient outcomes. It’s varied, rewarding work that allows you to use your full clinical skillset while helping patients access the right care, at the right time.

At IC24, we’re looking for an Advanced Clinical Practitioner (ACP) to join our East Sussex Home Visiting Service. This is an opportunity to work at the forefront of community-based care, delivering high-quality clinical assessments and treatment to patients in their own homes while supporting wider urgent and primary care services.

As an experienced clinician, you'll work autonomously within your professional scope of practice, providing assessment, diagnosis, treatment and prescribing for patients presenting with acute, urgent and undifferentiated conditions. You'll play a key role in helping patients receive the right care in the most appropriate setting, reducing unnecessary hospital admissions and improving patient outcomes.

This role forms part of our East Sussex Home Visiting Service, supporting patients across the community. You'll work as part of a highly skilled multidisciplinary team delivering responsive care where it is needed most, helping patients avoid unnecessary hospital attendance wherever appropriate.

To Be Successful In This Role, You'll Need
