Job summary

Practice Plus Group is seeking experienced Advanced Nurses or Clinical Practitioners to join our Out of Hours team, delivering high-quality care outside regular GP hours across the UK.

We offer flexible shift patterns and autonomous clinical decision-making within a multidisciplinary team to ensure patient-centered care and safe outcomes. DBS checks and references are required as part of the employment process.

Qualifications

• Registered Nurse with MSc in Advanced Clinical Practice or equivalent.
• Holder of V300 / Non-Medical Prescribing qualification and relevant post-qualification experience.

Responsibilities

• Deliver comprehensive clinical assessment, diagnosis, and treatment for urgent care needs.
• Conduct face-to-face consultations from a clinical base in a fully equipped environment.
• Assist with telephone triage when fewer face-to-face consultations are booked.
• Manage telephone triage, provide clinical advice and direct to appropriate care pathways.
• Work autonomously while collaborating with a multidisciplinary team.
• Maintain accurate clinical records and adhere to policies and NHS guidelines.

Job description

Are you an experienced Advanced Nurse or Clinical Practitioner passionate about delivering high-quality patient care? Do you thrive in a fast-paced, dynamic healthcare environment? Practice Plus Group is seeking dedicated healthcare professionals to join our Out of Hours team and make a real difference in the community. Practice Plus Group is a leading provider of urgent care services across the UK, committed to delivering safe, effective, and compassionate care to patients when they need it most outside of regular GP hours. For a full list of our locations please ClickHere We can offer a range of sessions to allow you to flexibly choose the right shift pattern for you. In OOH we assess and treat patients who require urgent unscheduled intervention suitable for treatment by general practice clinicians when the GP surgeries are closed. Our referrals come from either 111, via a dedicated health care professional line used by paramedics and district nursing teams or from the AED or MIU.

• Deliver comprehensive clinical assessment, diagnosis, and treatment for patients presenting with urgent healthcare needs.
• Conduct consultations from the clinical base, providing face-to-face care in a fully equipped environment.
• Assisting with telephone triage calls when there are lower levels of face-to-face consultations booked.
• Manage telephone triage, providing expert clinical advice, risk assessment, and directing patients to appropriate care pathways.
• Work autonomously while collaborating closely with a multidisciplinary team to ensure seamless, patient-centred care.
• Maintain accurate clinical records and adhere to Practice Plus Group policies and NHS guidelines to uphold high standards of care.

• Registered Nurse with MSc in Advanced Clinical Practice or equivalent.
• Experience within Out of Hours, Primary Care GP Surgery, or Urgent Treatment Centre.
• Holder of V300 / Non-Medical Prescribing qualification and minimum 6 months experience post qualification.
• Ability to autonomously see and treat patients of all ages.
• Confident clinical decision-maker with strong assessment skills.
• Excellent communicator with a patient-centred approach.
• Flexible, self-motivated, and committed to ongoing professional development.
